Within the scope of the Turkish Personal Data Protection Law No. 6698, I declare that I have read and understood the Privacy Notice prepared by the data controller, Dietitian Elif Arslan.
Accordingly, I give my explicit consent for my special category personal data (health data) that I share of my own free will through the appointment form, email, phone or during sessions, such as:
- my height, weight and other body measurements,
- any illness, allergy and intolerance information,
- medications and supplements I use,
- my laboratory results,
- my pregnancy and breastfeeding status and eating habits,
to be processed for the purposes of:
- scheduling my appointment,
- assessing my nutritional status and preparing a personalised nutrition plan,
- conducting follow-up sessions,
and to be stored only to the extent required by these purposes.
I understand that, if I give separate consent for this, relevant information may be shared with my physician so that my nutrition plan is consistent with my physician's recommendations.
I understand that I may withdraw my consent at any time by notifying kvkk@elifarslan.example in writing. Withdrawal of consent does not affect the lawfulness of processing carried out before the withdrawal.
